Optimize medication therapy as a Pharmacist with CGMH, using patient assessments and monitoring across 8-hour and 12-hour shifts. This is permanent part time work.

As a Pharmacist in CGMH’s Pharmacy department, you will support patient-centered pharmaceutical care and improved patient outcomes. You will collaborate with interdisciplinary teams, review laboratory information, verify orders, and contribute to medication safety. The position also includes education, mentoring, and quality improvement initiatives.

Key Responsibilities:

- Complete patient medication assessments and optimize medication therapy
- Review laboratory information and oversee therapeutic drug monitoring
- Verify physician orders for appropriateness, interactions, and contraindications
- Participate in clinical rounds and antimicrobial stewardship activities




- Educate healthcare providers, nursing staff, learners, and pharmacy students

Requirements:

- Registered Pharmacist in good standing with Ontario College of Pharmacists
- Bachelor of Science in Pharmacy or Doctor of Pharmacy
- Knowledge of evidence-based pharmacotherapeutics and pharmacokinetics
- Knowledge interpreting laboratory and clinical monitoring parameters
- Strong organization, communication, problem-solving, and decision-making skills
