Extendicare in Carleton Place, Ontario, is seeking a dedicated RAI-MDS Coordinator to lead the resident assessment process across the interdisciplinary team. You'll ensure timely, accurate MDS data informs individualized care plans and supports quality improvement.

You'll coach staff on RAI-MDS, InterRAI LTCF, and related systems, and collaborate with residents, families and colleagues to drive excellence in care.
