Deliver occupational therapy in Sudbury home care, using clinical reasoning and evidence-informed interventions. Support older adults transitioning safely from hospital to home. As an Occupational Therapist with Bayshore Integrated Care Solutions, you will assess, treat, educate, and manage clients throughout recovery. You will work within an interdisciplinary care model supporting independence, safety, and quality of life. The position is an independent contractor assignment serving clients in their homes.

Key Responsibilities
- Complete comprehensive occupational therapy assessments
- Develop individualized treatment plans, goals, and discharge recommendations
- Evaluate and modify treatment plans as client needs change
- Communicate findings and education to clients, families,



and caregivers
- Maintain accurate clinical documentation using shared client records

Requirements:
- Master's degree in Occupational Therapy or recognized equivalent
- Registration in good standing with COTO
- Strong clinical reasoning and decision-making abilities
- Proficiency with electronic documentation and secure digital communication
