Support older adults returning home as an Occupational Therapist with Bayshore Integrated Care Solutions across Ontario. Apply clinical reasoning within an interdisciplinary care model. This independent contractor position provides Occupational Therapy services in clients’ homes. You will assess, treat, educate, and manage clients throughout recovery. The work supports safety, independence, quality of life, and meaningful outcomes after hospital discharge. Collaboration occurs through one shared client record.

Key Responsibilities
- Complete comprehensive occupational therapy assessments
- Develop individualized treatment plans, goals, and discharge recommendations
- Evaluate and modify treatment plans as client needs change
- Communicate findings and education to clients,



families, and caregivers
- Maintain timely clinical documentation

Requirements:
- Master’s Degree in Occupational Therapy or recognized equivalent
- Registration in valuable standing with COTO
- Strong clinical reasoning and decision-making abilities
- Proficiency with electronic documentation and secure digital communication
