Nurse Practitioner
- Primary Care Shift: 08:00am-04:00pm, Monday to Friday Description: We are seeking a Nurse Practitioner to provide comprehensive primary care to youth, approximately ages 12–25, in a community-based setting.
The NP works autonomously and within an interprofessional team to deliver accessible, youth-friendly, trauma-informed care.
The focus is on early intervention, health promotion, and connecting young people to the services they need.

Key Responsibilities
Provide primary care including health assessments, diagnosis, treatment, and ongoing management of acute and chronic conditions.
Order and interpret diagnostic tests, prescribe medications, and make referrals within the NP scope of practice.
Deliver sexual and reproductive health services, including contraception, STI testing and treatment, and pregnancy counselling.
Screen for and support mental health and substance use concerns, collaborating with various professionals.
Provide immunizations, preventive care,



and health education tailored to adolescents and young adults.
Address issues of consent, confidentiality, and capacity appropriately for youth clients.
Advocate for clients and coordinate with schools, families (where appropriate), and community and social services.
Maintain accurate, timely clinical documentation in the electronic medical record (EMR).

Qualifications:
Current BLS certification and professional liability protection.
Knowledge of youth mental health, harm reduction, and affirming care.
Strong communication skills and a non-judgmental, client-centered approach.
Clear Vulnerable Sector Check.
French or additional language skills are an asset. Work Setting: Community health center, youth wellness hub, or school-based clinic setting; may include evening hours and outreach.
