We are currently seeking a dedicated, compassionate, and reliable Home Support Worker to provide individualized care to a client in Listowel, Ontario.

Position Summary

The Home Support Worker will be responsible for assisting a client with daily living activities, personal care, household responsibilities, mobility, and general well-being.

The successful candidate must be patient, professional, attentive, and capable of providing person-centred care while following an established care plan and maintaining a safe, comfortable home environment.

Key Responsibilities & Job Duties1. Personal Care & Hygiene Assistance

- Assist with daily personal hygiene, including bathing, showering, and sponge baths.
- Support dressing, grooming, oral hygiene, and other personal care routines.
- Provide toileting assistance, including incontinence care when required.
- Assist with getting in and out of bed and preparing for daily activities.
- Maintain the client's comfort, privacy, dignity, and personal preferences.
- Observe and report changes in skin condition, hygiene needs, or physical well-being.

2. Mobility & Physical Assistance

- Assist the client with safe movement throughout the home.
- Support walking, standing, sitting, and repositioning as required.
- Assist with transfers between bed, chair, wheelchair, or other mobility equipment, when trained and authorized.
- Encourage safe mobility and independence within the client's abilities.
- Monitor environmental hazards and implement fall-prevention precautions.
- Follow proper body mechanics and safe transfer procedures.

3. Meal Preparation & Nutritional Support

- Prepare breakfast and other simple, nutritious meals based on the client's preferences and dietary requirements.
- Assist with meal setup, serving, and feeding when necessary.
- Encourage appropriate fluid intake and regular meals.




- Clean the kitchen and dining area after meal preparation.
- Monitor and report changes in appetite or eating habits.

4. Light Housekeeping & Home Management

- Maintain cleanliness and organization in the client's immediate living areas.
- Perform light housekeeping, including sweeping, dusting, and wiping surfaces.
- Assist with laundry, folding clothes, and changing bed linens.
- Wash dishes and maintain kitchen cleanliness.
- Organize personal belongings and essential care supplies.
- Dispose of household waste and maintain a safe, hygienic environment.

5. Companionship & Emotional Support

- Provide friendly conversation, reassurance, and meaningful companionship.
- Encourage participation in appropriate daily activities.
- Support the client's emotional well-being and social engagement.
- Respect individual routines, cultural preferences, and personal choices.
- Promote independence and a positive home environment.

6. Medication Reminders & Health Monitoring

- Remind the client to take prescribed medications according to the established care plan.
- Observe and report any changes in physical condition, behaviour, mood, or mobility.
- Monitor for signs of discomfort, unusual fatigue, or potential safety concerns.
- Promptly communicate significant observations to the designated care coordinator.
- Do not administer medications or perform regulated clinical procedures unless legally permitted, appropriately trained, and specifically authorized.

7. Care Documentation & Reporting

- Follow the individualized client care plan and assigned instructions.
- Accurately record completed duties and relevant daily observations.
- Maintain timely and accurate electronic shift notes.
- Complete required clock-in and clock-out procedures for every scheduled shift.
- Immediately report falls, injuries, incidents, or changes in the client's condition.
- Communicate professionally with the Aviora Healthcare coordination team.
- Maintain strict confidentiality regarding client and family information.

8. Safety, Infection Prevention & Professional Conduct

- Follow standard infection prevention and control practices.
- Use personal protective equipment when required.
- Maintain a clean and safe caregiving environment.
- Follow all company policies, care protocols, and emergency procedures.




- Arrive punctually and maintain dependable attendance.
- Respect professional boundaries and the client's rights.
- Immediately report unsafe conditions, suspected abuse, or neglect through established reporting procedures.

Qualifications & Requirements

Required:

- Previous experience providing home support, personal care, senior care, or related caregiving services.
- Ability to assist with activities of daily living safely and compassionately.
- Strong communication, interpersonal, and time-management skills.
- Ability to follow written care plans and instructions.
- Reliability, punctuality, professionalism, and attention to detail.
- Ability to work independently in a private home setting.
- Willingness to complete a Vulnerable Sector Check.
- Legal authorization to work in Canada.
- Availability for scheduled morning shifts in Listowel, Ontario.

Preferred:

- Personal Support Worker (PSW), Home Support Worker (HSW), Health Care Aide, or equivalent training/certification.
- Current First Aid and CPR certification.
- Previous experience supporting older adults or individuals with mobility limitations.
- Experience with dementia care, fall prevention, and safe mobility assistance.
- Access to reliable transportation.
