Modern Angels Healthcare is seeking compassionate and experienced Registered Nurses (RN) and Registered Practical Nurses (RPN) to provide community-based palliative and end-of-life care services across the Halton Region. This role offers an opportunity to make a meaningful difference in the lives of patients and families by delivering high-quality, person-centered care in home and community settings.
Key Responsibilities
- Provide comprehensive nursing care to palliative and end-of-life patients in the community.
- Conduct health assessments and monitor patient status.
- Support symptom management, pain control, and comfort measures.
- Collaborate with physicians, nurse practitioners, care coordinators, and interdisciplinary healthcare teams.
- Educate patients and families on disease progression, treatment plans, and end-of-life care.
- Maintain accurate and timely clinical documentation.
- Participate in care planning and case conferences as required.
- Ensure care is delivered in accordance with professional standards and best practices.
Qualifications/ Mandatory Requirements
- Current registration in good standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
- Minimum one (1) year of recent community palliative care experience.
- LEAP Certification (Learning Essential Approaches to Palliative and End-of-Life Care).
- Current CPR/BLS Certification.
- Valid Ontario Driver's License and access to a reliable vehicle.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently in a community setting.
Preferred Qualifications
- Experience providing home and community care services.
- Knowledge of palliative care resources within Halton Region.
- Experience with electronic charting and documentation systems.
