About the Role

Senior Circle Support Inc. is urgently seeking an experienced Registered Practical Nurse (RPN) to provide peritoneal dialysis care to patients in their homes throughout Durham Region.

We are looking for a nurse with recent, hands-on peritoneal dialysis experience who can confidently provide patient care with minimal supervision.

This is a direct patient care position, not an educator or training role.

Responsibilities

- Provide in-home peritoneal dialysis nursing care according to prescribed orders and individualized care plans.
- Perform automated peritoneal dialysis (APD) cycler setup, connection, disconnection, and tear-down.
- Perform continuous ambulatory peritoneal dialysis (CAPD) procedures as prescribed.
- Monitor vital signs, weight, fluid balance, ultrafiltration, and patient condition.
- Recognize and promptly escalate abnormal findings, cloudy effluent, suspected infection, and other treatment concerns.
- Follow infection prevention and aseptic technique requirements.
- Administer prescribed medications within professional scope and organizational policies.




- Maintain accurate and timely nursing documentation.
- Monitor dialysis supplies and communicate shortages promptly.
- Communicate effectively with patients, families, the care coordination team, and designated clinical contacts.

Required Qualifications

- Current registration as an RPN with the College of Nurses of Ontario (CNO), with entitlement to practise.
- Hands-on peritoneal dialysis experience is an assest.
- Experience performing APD cycler setup and tear-down.
- Solid understanding of PD procedures, infection prevention, and patient monitoring.
- Ability to work independently in a community home care setting.
- Strong clinical judgment, reliability, and communication skills.
- Ability to travel between patients' homes throughout Durham Region.

Preferred Qualifications

- Experience with both APD and CAPD.
- Familiarity with Vantive/Baxter peritoneal dialysis equipment.
- Previous home care or community nursing experience.

Compensation

- $30 per hour.
- Morning, evening, and weekend availability is an asset.
