LPN Case Manager Location: Ontario, CA Schedule: Monday-Friday | 8:00 AM-5:00 PM Position Summary Seeking an experienced LPN Case Manager to manage a caseload of up to 150 patients.
This role focuses on patient outreach, assessments, care coordination, health education, and reducing avoidable ED visits and hospitalizations.
Key Responsibilities Conduct daily patient and caregiver outreach.
Complete patient assessments and care coordination.
Educate patients on health management and available resources.
Build robust patient relationships and promote independence.
Maintain accurate and timely documentation.
Manage a caseload of up to 150 patients.
Requirements Current and unrestricted LPN license.
Strong communication and patient engagement skills.
Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
