Apply addictions knowledge and case management skills with The East Toronto HART Hub, supporting residents through a full time, 12-month contract. Work 37.5 hours weekly.
The Case Coordinator contributes to evidence-based healthcare, treatment, social services, and housing support. You will work with individuals experiencing drug use, mental health, housing, and related concerns. Responsibilities include assessment, supportive counselling, brief intervention, care planning, urine screening, group facilitation, and referral coordination.
Key Responsibilities: Complete assessments, care plans, and case management activities
Deliver supportive counselling and brief therapeutic interventions
Facilitate psychoeducational groups for residents




Coordinate referrals with community and allied service partners
Provide information and health promotion
Requirements: Experience working with men affected by addictions or concurrent disorders
Demonstrated case management experience
Experience using motivational interviewing and therapeutic approaches
Knowledge of addiction, trauma, recovery, and mental health
Eligibility for regulated health profession membership
Bring assessment, motivational interviewing, and addiction service experience to case coordination at The East Toronto HART Hub.
