Support patient transitions at Providence Care Hospital using social assessments, navigation, and team-oriented discharge planning. This temporary part-time role serves patients and families.
Reporting to the Director, Patient Flow, the Social Service Worker coordinates social service support across Providence Care Hospital and Providence Transitional Care Centre. You will collaborate with interprofessional teams, community partners, patients, and families. The work focuses on assessment, individualized transition planning, service coordination, and timely hospital transitions.
Key Responsibilities: Conduct comprehensive patient and family assessments
Coordinate services for referred patients
Develop individualized transition plans collaboratively




Identify supports and community service linkages
Monitor progress and revise plans as required
Requirements: Social Service Work diploma
Active unrestricted Ontario practice license required
License with the College of Social Workers and Social Service Workers
One year hospital navigation experience preferred
One year transition and discharge planning experience preferred
Apply social service expertise to patient navigation, coordinated transitions, and discharge planning at Providence Care’s hospital sites.
