ParaMed seeks a full time Licensed Practical Nurse for visits across community settings. You will care for patients of all ages and varying complexity, with Registered Nurse support for more complex patients. The work focuses on helping patients remain comfortable at home.
Key Responsibilities: Complete scheduled community visits within your nursing scope
Provide ethical and protected nursing care
Assess individual patient needs using appropriate tools
Monitor care plans and adjust them as required
Maintain professional records of encounters and plans
Requirements: Active CLHA registration as a Licensed Practical Nurse
Valid driver’s license and access to a vehicle
Willingness to travel between patient residences
Experience with vulnerable populations
Required on call coverage, including after hours
Support patients at home through community nursing, careful assessment, care planning, and dependable Licensed Practical Nurse practice.
