Key Responsibilities: Signpost residents to appropriate local support services
Support access to health, wellbeing, and social inclusion activities
Assist unpaid carers with relevant services and resources




Participate in practice-based multidisciplinary meetings and case discussions
Monitor referrals and support timely assessments and services
Requirements: Knowledge of borough services, resources, and eligibility criteria
Ability to create personalised support plans
Ability to work with volunteers using established procedures
Ability to recognise escalation and urgent intervention needs
Ability to support informed care and resource decisions
