Covenant Health is recruiting a Social Worker II to join the Emergency Department Clinical Triage Team on a one-year pilot. You will support timely psychosocial assessment, brief intervention and resource navigation for patients in the ED.
Work collaboratively with the TLP/NP/RN and broader ED team to identify care pathways, address social determinants of health, and facilitate timely transitions. Two part-time positions are available for this quick-paced, team-based model.
