Canadian Mental Health Association Lambton Kent is seeking a Clinical Case Manager to support adults with mental health challenges at the Sarnia site. The role focuses on client-centered planning, recovery, and empowerment, with flexible schedule and a 35-hour work week.

Reporting to the Manager of Integrated Services, you will perform assessments, develop care plans, coordinate services, and liaise with families and community supports to promote safe community living.
