University Health Network (UHN) is seeking a Home Hemodialysis Personal Support Worker to support patients at home and in the community. The role emphasizes protected, comfortable dialysis delivery and comprehensive client care within a busy, patient-centered program.

You will work under nursing delegation, manage supplies, document care, and assist with daily living activities, while adapting to evolving program needs and patient volumes.
