Southern Health-Santé Sud in Steinbach is hiring a Case Coordinator to join the Home Care Program. You will work within a multidisciplinary team to assess plans, coordinate services and monitor outcomes for clients in our community.

Reporting to the Manager Health Services, you will exercise independent judgment, manage priorities, and ensure care is client-centered and aligned with organization policies. This in-person role requires flexibility for days, evenings and weekends.
