The community program includes 20 transitional care beds and operates seven days weekly. Key Responsibilities:
- Assist individuals transitioning from hospital to community

Requirements:
- High School Diploma required
- Personal support experience with seniors or disabled people
- Personal support, first aid, or healthcare certification preferred
- Planning, organization, and effective problem-solving abilities
- Solid oral and written English communication skills
