This Personal Support Worker position serves a roster of clients through a diverse, self-directed care team. You will contribute to client-centered planning across physical, cognitive, social, emotional, and spiritual domains. The team connects care with client goals and supports improved transitions home.

Key Responsibilities:

Requirements:

- Valid Personal Support Worker certificate, RN, or RPN
- Foreign-trained nurse healthcare professional accepted
- Required immunizations, including MMR, Varicella, TB, Tetanus, Covid
- Valid driver’s license and work vehicle
- Physical ability to lift approximately fifty-five pounds
