Impact Life seeks a dual-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner. You will support the Rural Health Hub and Integrated Outpatient Services for people with substance use disorders, behavioral health conditions, primary care needs, and complex social needs. The position delivers low-barrier, whole-person care.

Key Responsibilities:

- Conduct comprehensive medical and psychiatric health assessments
- Diagnose and treat acute and chronic conditions
- Provide primary and preventive healthcare services




- Prescribe and manage MOUD and psychotropic medications
- Coordinate care with interdisciplinary and community partners

Requirements:

- Current unrestricted Delaware Nurse Practitioner license
- Dual FNP and PMHNP certification required
- ANCC, AANP, or equivalent board certification
- Active DEA registration and prescriptive authority
- At least two years of relevant clinical experience
