Working for Vancouver Island Health authority, this casual position supervises Community Health Workers and coordinates client care. You will conduct home visits, implement Care Plans, respond to service delivery concerns, and collaborate with clients, families, and the health care team. The work follows a 24-hour, seven-day model.

Key Responsibilities:

- Coordinate delivery of Home Support Services
- Conduct home visits and implement client Care Plans
- Respond to service delivery concerns
- Supervise, train, evaluate, and discipline Community Health Workers
- Collaborate with clients, families, and health care teams

Requirements:

- Practice registration with BC College of Nurses and Midwives
- WHMIS, CPR, Standard First Aid, and Food Secure certification
- Valid BC Driver's License and reliable vehicle
- Clear required criminal record checks
- Computer, communication, organization, and conflict resolution skills
