Lead virtual acute care coordination as a Registered Nurse with Southlake Health in Newmarket, Ontario, supporting patients receiving hospital-level care at home.

This temporary part time Registered Nurse position supports the Acute Care @ Home virtual unit. You will oversee census, acuity, clinical risk, resources, admissions, transitions, and escalation. The work requires emergency nursing experience, acute-care assessment, critical thinking, and clinical decision-making. You will coordinate interdisciplinary care across hospital and home settings.

Key Responsibilities:

- Provide daily clinical and operational oversight of the virtual unit
- Coordinate admissions, transitions, handovers, and urgent hospital transfers
- Respond to deterioration and escalate changing clinical needs




- Coordinate interdisciplinary staff, physicians, patients, and families
- Lead huddles, coaching, pathways, and quality improvement activities

Requirements:

- Current Ontario Registered Nurse registration in good standing
- Minimum three years of recent Emergency Department nursing experience
- Strong acute-care assessment and critical-thinking skills
- Ability to coordinate complex care across disciplines
- Ability to manage competing priorities and adjust resources
