Coordinate complex patient care remotely for Spartan Wellness using bilingual communication, clinical documentation, and organized follow-up across Canada. Support patients and providers throughout each care journey.

As a full-time, permanent Clinical Care Coordinator, you will manage patient casework rather than employees. You will work with Nurse Practitioners, patients, insurers, and external healthcare providers. The work includes reviewing medical records, prescriptions, diagnoses, treatment plans, and insurance information. Your coordination keeps appointments, referrals, labs, documentation, and follow-ups on track.

Key Responsibilities:

- Serve as a patient contact throughout each care journey
- Track appointments, referrals, labs, prescriptions, and clinical documentation
- Coordinate consultations, treatment providers,



and Independent Medical Examinations
- Review records and update digital patient information accurately
- Follow up with patients, providers, and insurers on missing documentation

Requirements:

- Fluency in English and French
- Healthcare coordination or clinical administration experience
- Understanding of medical terminology and clinical documentation
- Strong organization, multitasking, and written communication skills
- Comfort working independently in a fully remote workplace
