Support a young client transitioning home from hospital through individualized nursing care, clinical monitoring, and collaboration with family and healthcare professionals. Key Responsibilities:

- Deliver skilled nursing care in the client’s home
- Administer medications and provide G-tube feeding care
- Monitor and document condition, treatment response, and health changes
- Assist mobility, transfers,



positioning, and prescribed splint use
- Provide personal care and daily living support

Requirements:

- Graduate from an accredited or recognized School of Nursing
- Current LPN registration in positive standing
- Valid CPR certification
- Pediatric nursing experience considered a strong asset
- Strong assessment, critical thinking, and communication skills
