Durham Community Health Centre is seeking a Casual/On-call Case Worker for the HART Hub in Whitby, Ontario. You will join a 24/7 stabilization team, building trusting relationships with residents, supporting daily needs, and contributing to individualized housing and stabilization plans.

The role emphasizes relationship, safety, dignity, and consistency as foundations of care. Collaborate with Case Management, Housing, Primary Care, Addictions and leadership to ensure coordinated support;
