Halton Healthcare seeks an RPN for its Home Dialysis Assistance Program. You will provide assessment, education, decision support, and follow-up care for patients and families. The work includes home visits, dialysis management, medication administration, and collaboration with the renal interdisciplinary team.
Key Responsibilities:
- Assess patients receiving home peritoneal dialysis
- Educate patients and families about home dialysis
- Coordinate appointments and community home visits
- Prepare circuits and program dialysis machines
- Monitor treatments and provide interventions with RN collaboration
Requirements:
- Current College of Nurses of Ontario registration in valuable standing
- Current BCLS certification at time of hire
- Experience processing physician orders
- Access to a vehicle for community transportation
- Ability to practice fully within this environment
