Lead clinical care coordination with CBI Home Health across Winnipeg and Portage La Prairie, supporting comprehensive assessments and individualized service plans. This full time, on-site position requires highway driving.

As Clinical Supervisor, you will oversee individual care for assigned service recipients and support high-quality, best-practices outcomes. Your work includes intake assessments, risk assessments, care planning, documentation review, and ongoing monitoring. You will collaborate with clinical, operational, and external stakeholders to coordinate services and address concerns.

Key Responsibilities:

- Assist with case management and clinical care coordination
- Support intake, risk assessments, and individualized care plans
- Monitor care, documentation, goals,



and service quality
- Coordinate appointments, equipment, and stakeholder communications
- Participate in case conferences and clinical escalation

Requirements:

- BCBA, LPN, RPN, RN, or allied health degree in progress
- First Aid and CPR certification
- Minimum one year supporting disabilities and complex service needs
- Full driver’s licence and vehicle
- Strong leadership, clinical, communication, and organizational skills
