Interlake-Eastern Regional Health Authority is seeking a Home Care Case Coordinator to coordinate a broad range of professional services for clients in the Ashern area. You will ensure client-focused planning, assessment, and coordination of care across home care providers and community agencies.

The role requires licensure as per skilled designation, experience with home care assessment systems, and a valid Manitoba driver’s license. Travel within the region may be required.
