The Interlake-Eastern Regional Health Authority seeks a Home Care Case Coordinator for its non-clinical Home Care department. This position coordinates professional and non-professional services through a client-driven process. It assesses, plans, implements, monitors, and evaluates options meeting health and human service needs.

Key Responsibilities:

- Coordinate qualified and non-professional home care services
- Assess client health and human service needs
- Plan and implement appropriate service options
- Monitor and evaluate client services and resources
- Liaise with agencies, facilities, clients, and family caregivers

Requirements:

- Required BSW, OT, PT, RPN, RN, LPN, RD, or related degree
- Required professional licensure or registration in good standing
- Preferred two years’ community-based care experience
- Experience with elderly and palliative care clients
- Valid Manitoba driver’s licence and vehicle required
