South West London and St Georges Mental Health Trust is seeking a Frailty Primary Mental Health Practitioner to carry out comprehensive biopsychosocial assessments in patients’ homes and community settings.

You will assess mood, anxiety, psychosis, cognitive change, BPSD and delirium, develop safety/crisis plans with families and professionals, and support safeguarding processes. The role includes care planning and evidence-based interventions.
