As a Registered Practical Nurse, you will provide care across community settings for patients of all ages and varying complexity. You will work within your scope, with Registered Nurse support for more complex patients. This position supports comfort and continued home living.

Key Responsibilities:

- Complete scheduled community visits within your scope of practice
- Provide ethical and safe nursing care
- Assess individual patient needs using appropriate techniques
- Execute, monitor, and adjust care plans as required
- Maintain qualified records for patient encounters and plans

Requirements:

- Registered Practical Nurse registration with CNO in good standing
- Valid driver’s license and access to a vehicle
- Willingness to travel between patient residences
- Reliability and flexibility
- Required after-hours on-call coverage
