Support residents at AgeCare through professional nursing care, physician liaison, and direction for Personal Support Workers.

This Registered Practical Nurse position supports AgeCare policies, resident care needs, and communication across the health care team. You will liaise with physicians and external health care partners while collaborating with Registered Nurses. The position also requires shift reporting and timely communication about resident status.

Key Responsibilities:

- Liaise with physicians and external health care partners
- Collaborate with health care team members on resident needs
- Notify physicians when resident status changes significantly
- Provide direction to Personal Support Workers
- Contribute input to residence and unit procedure revisions

Requirements:

- Background in geriatric or long-term care nursing
- Knowledge of rehabilitation nursing and activation
- Knowledge of personnel administration in nursing settings
- Current Ontario nursing Certificate of Competence
- Valuable standing with the College of Nurses of Ontario
