Fraser Health is hiring a Casual Hospital Community Outreach Counsellor, CYYA, within a multidisciplinary Transition Team. You will provide life skills training, coaching, advocacy, and support to clients and caregivers. Care is delivered through established treatment plans, trauma-informed approaches, and evidence-informed practice.

Key Responsibilities:

- Provide life skills training, coaching, advocacy, and support
- Use psycho-education, de-escalation, and behaviour management techniques
- Assist with home visits, groups, outreach, and client meetings
- Support appropriate medication management under established treatment plans




- Facilitate attendance at appointments, programs, and services

Requirements:

- Baccalaureate degree in an appropriate health-related discipline
- One year recent multidisciplinary experience with youth mental health
- Demonstrated ability to support mental health or substance use needs
- Knowledge of behaviour management strategies and techniques
- Ability to communicate effectively verbally and in writing
