0.8 FTE – 1.0 FTE I Contract Position - Up to 22-months The Brockton and Area Family Health Team is seeking a motivated and collaborative Nurse Practitioner to join our interdisciplinary team at our Paisley and Chesley sites.

This is an excellent opportunity to provide comprehensive, patient-centred primary care while working alongside local family physicians and a dedicated team of health care professionals. The successful candidate will contribute to improving access to primary care and supporting the health and well-being of individuals and families in our communities.

Main Responsibilities:

- Provide comprehensive primary care services, including health assessments, diagnosis, treatment, prescribing, follow-up, consultation and referral.
- Work collaboratively with family physicians and interdisciplinary team members to coordinate care and address the diverse health needs of patients.
- Identify patient needs and connect individuals and families with appropriate community and healthcare resources.
- Participate in care planning, case discussions and collaborative approaches to complex patient needs.
- Maintain accurate and timely clinical documentation and ensure continuity of care.




- Contribute to quality improvement initiatives and the ongoing development of team-based primary care services.

Position Qualifications and Preferred Experience:

- Bachelor’s degree in Nursing.
- Current Certificate of Registration in the Extended Class from the College of Nurses of Ontario, and eligibility to practice as a RN-EC.
- Experience practicing as a NP in a Community Health or Primary Care setting.
- Proven ability to communicate effectively with patients, families, and members of an interdisciplinary team.
- Experience in delivering patient education and chronic disease management.
- Proficiency in the use of computers and healthcare information technology.
- Possession of a valid driver’s license and access to a reliable vehicle.

Pay: $64.10-$66.70 per hour
