About FORM
FORM Face + Body brings plastic surgery, medical aesthetics, hair restoration, men's health, women's health and our Life+ wellness division together on one campus at Yonge and Eglinton in Toronto. FORM's surgeons operate in their on-site, three operating room surgical centre, and every patient is guided by one team from first call to final result.
FORM Men's Health is led by its Medical Director, Dr. Noah Stern, a urologist with an expertise in men's health. The clinic offers men a private, modern place to deal with hormones, sexual and pelvic health, complete with an assessment, diagnostics and treatment options, all under one roof.
We are recruiting for a Nurse Practitioner to help grow the clinic. You will own the patient's clinical journey, from the first assessment through treatment and follow-up, working with our medical director, our pelvic floor physiotherapist and our patient care team. Full-time and part-time schedules are available.
What you will do
- Interact with new patients for comprehensive men's health assessments, including history, examination and goals
- Order and interpret bloodwork and diagnostics, and build individual treatment plans
- Manage testosterone and hormone optimization, including monitoring and follow-up
- Assess and treat erectile function and pelvic health concerns, and coordinate in-clinic therapies such as penile focused shockwave and Apex radiofrequency therapy
- Prescribe within your scope and refer to our medical director, physiotherapist or outside specialists when needed
- See patients for follow-up appointments, and keep complete, timely clinical notes
- Work with your Patient Care Coordinator so every patient leaves with a clear plan and next visit booked
What you bring
- Registration in good standing with the College of Nurses of Ontario as a Nurse Practitioner
- Preferred experience in men's health,
urology, sexual health, endocrinology or primary care, with an interest in men's health
- Comfort discussing sensitive topics with warmth, discretion and confidence, and delivering treatments with the same level of comfort
- Professional liability protection
- Strong clinical judgement and organized, timely documentation in an electronic medical record
- Fluent English. Additional languages are an asset

Schedule
- Monday to Friday, 8:30 am to 4:30 pm, on site (not remote)
