About FORM

FORM Face + Body brings plastic surgery, medical aesthetics, hair restoration, men's health, women's health and our Life+ wellness division together on one campus at Yonge and Eglinton in Toronto. FORM's surgeons operate in their on-site, three operating room surgical centre, and every patient is guided by one team from first call to final result.

We are growing and actively assessing strong candidates for a Patient Care Coordinator who loves the patient conversation as much as we do.

You will be the patient's guide across every FORM division. You will take inquiries from first contact to a booked consultation, support patients through their consultation, present treatment plans and pricing with confidence, and stay with them through treatment and follow-up. This is a sales-focused role built on trust, not pressure.

What you will do

- Respond quickly to new inquiries by phone, email, text and web, and book consultations
- Learn each patient's goals and match them with the right provider and division
- Join consultations, present treatment plans and pricing, and guide patients to a confident decision
- Follow up on open plans, consultations and post-treatment care so no patient is left waiting
- Introduce related treatments, programmes and skincare where they fit the patient's goals
- Work closely with our surgeons, physicians, nurse practitioners, nurses and aestheticians
- Keep accurate records in our patient management and CRM systems
- Track your pipeline and work toward monthly consultation, conversion and revenue goals

What you bring

- Two or more years in plastic surgery, medical aesthetics, dermatology or another premium patient-facing setting




- A track record in sales or patient coordination, with goals you met and exceeded
- Warm, polished communication in person, on the phone and in writing
- Fluent English. Additional languages are an asset
- Comfort with patient management software, CRM tools and Microsoft 365
- Sound judgement with confidential health information and an understanding of patient privacy
- Strong organizational skills and follow-through on delegated tasks and responsibilities

Schedule Monday to Friday, 8:30am to 4:30pm, In-Person (Not Remote)
