About Solve Health

Solve Health is a Toronto-based family office for health. A small number of families, including their children, pay an annual membership for a physician-led team that supports their health as a whole, year-round. One record. One plan. Follow-through until every action is done.

Insured care stays with OHIP and each family's own doctors. Solve adds the planning, the coordination, and functional and longevity medicine on top. The clinical team has a family physician as medical director, a pediatrician, a naturopathic doctor who leads functional medicine, and a physician who designs longevity protocols.

Responsibilities

- Be each family's first call. Triage concerns within RN scope
- Act on the medical directives the physicians write
- Bring a physician in quickly, with a short, clear summary they can act on
- Keep each family's record together: the summary before each review, the action list after it, and follow-up until every item is closed
- Track screening and vaccines for every family member, including children
- Provide basic hands-on care, including weights,



measurements, and blood pressure, in homes and in clinic
- Explain care plainly to families and stay with them until each issue is resolved
- Keep total discretion. Hold clinical boundaries politely with demanding families

Qualifications

- Current registration as an RN with the College of Nurses of Ontario, in good standing, with no findings, conditions, or restrictions
- At least three years of RN practice
- Experience in triage, primary care, outpatient pediatrics, nurse navigation, or private nursing is an asset
- Sound judgment on what to handle yourself and what to escalate
- Transparent, plain communication with families and physicians
- Openness to feedback and willingness to adjust how you work
- Strong follow-through: results chased, calls returned, loops closed
