Provide personal care and home support in person with Southern Health-Santé Sud as a Home Care Attendant in Portage la Prairie. Support clients across home settings.

This permanent, 0.6 FTE Home Care Attendant works under the Resource Coordinator in the Home Care program. You will assist frail, physically disabled, and mentally challenged clients of all ages. Services follow established plans of care and support clients remaining safely at home. The schedule includes days, evenings, and weekends.

Key Responsibilities:

- Provide personal care to assigned home care clients
- Deliver home support services according to care plans
- Assist clients remaining in their own homes safely
- Determine work priorities and methods independently
- Travel to regional facilities when duties require

Requirements:

- Grade 12 education is required
- Home Care Attendant or Health Care Aide Certificate
- Experience with elderly or disabled clients preferred
- Valid Class 5 licence and insured vehicle required
- Respect confidentiality and cultural diversity
