The Home Care Case Coordinator works with Interlake-Eastern Regional Health Authority at the Arborg Community Health Office. This non-clinical position coordinates professional and non-professional services while assessing, implementing, monitoring, and evaluating client needs. The work involves agencies, facilities, clients, and family caregivers.
Key Responsibilities: Assess client health and human service needs
Plan and implement appropriate home care options
Coordinate services using available community resources
Monitor and evaluate client service requirements




Liaise with agencies, facilities, and family caregivers
Requirements: Required BSW, OT, PT, RPN, RN, LPN, RD, or related degree
Required professional licensure or registration in valuable standing
Two years of community-based care experience preferred
Experience with interRAI HC, Electronic Home Care Record, and Relias
Valid Manitoba driver's licence and vehicle required
