As a Geriatric Support Services Coordinator, you will function within the Primary Care Team for members in a Senior Care Option plan. Coordinate contracted and community services while monitoring health, functional status, wellness, and independence. Conduct reassessments and connect members with public advantages.

Key Responsibilities:

- Function as a case manager within the Primary Care Team
- Make recommendations for contracted and community services
- Coordinate and implement member service plans
- Monitor health and functional status
- Conduct periodic reassessments of service plan appropriateness

Requirements:

- BA or BS in Social Work, Human Services, or related field
- Case management experience with geriatric populations
- Spanish and English fluency
- Valid driver’s license or reliable transportation
- LSW is a plus
