Key Responsibilities: Deliver patient-centric case management within program protocols
Provide therapy education, onboarding, adherence support, and follow-up
Coordinate care with physicians, clinics, pharmacies, and internal teams
Support funding, prior authorizations, and patient access solutions




Document interactions and report adverse events appropriately
Requirements: Diploma or degree in Nursing
Current unrestricted Alberta nursing registration in good standing
Experience providing patient care, education, coordination, or case management
Solid English communication and Microsoft Office proficiency
Valid driver's license and reliable transportation
