Key Responsibilities: Assess patient, resident, and family needs and strengths
Develop and update individualized plans of care
Provide nursing care and evaluate patient outcomes

Support admissions, discharges, transfers, and protected environments
Requirements: Graduate of a recognized practical nursing program
Current BCCNM practicing registration as a Licensed Practical Nurse
Ability to administer medications within full scope
Effective verbal and written communication skills
Physical ability to carry out position duties
