Support patients through social medicine services at UHN while engaging interdisciplinary teams and managing day-to-day social needs. This permanent full time role works days across UHN sites and community locations.
The Social Medicine Patient Support Representative is an integral member of the Social Medicine clinical team. Reporting to the Senior Manager, the representative engages patients and supports the interdisciplinary team. The position coordinates with the Social Medicine clinical team and partners to address patient social needs.
Key Responsibilities: Support the interdisciplinary Social Medicine clinical team
Engage patients throughout their Social Medicine care
Manage patients’ day-to-day social needs




Coordinate with the Social Medicine clinical team
Work with partners supporting patient needs
Requirements: Ability to support patients experiencing social needs
Ability to engage patients in clinical services
Ability to support interdisciplinary clinical teams
Ability to coordinate with team members and partners
Availability for permanent full-time day shifts
Bring patient engagement, social needs management, and interdisciplinary coordination to UHN’s Social Medicine clinical team.
