Support supported housing clients through assessment, transition planning, and housing consultation as a Social Worker II with AMH Supported Living.
Reporting to the Manager and leadership, you will serve clients with severe and persistent mental illness who require supported housing. The multidisciplinary outreach team includes Social Work, Nursing, Outreach Workers, and Psychiatry. You will provide case management, rehabilitation, support services, and education while applying recovery, harm reduction, trauma informed care, and client and family centered care principles.
Key Responsibilities: Support client transitions into successful community living
Provide housing consultation to clients, families, and stakeholders
Manage cases and coordinate treatment and support services




Participate in medication management and CTO management
Provide coverage for other team members as needed
Requirements: Three to five years adult mental health experience within five years
Knowledge of housing resources and mental health housing barriers
Knowledge of Mental Health Legislation and Community Treatment Orders
Experience conducting psychosocial assessments and treatment planning
Ability to work independently and collaboratively within multidisciplinary teams
Bring housing knowledge, case management experience, and sound judgment to supported living services for Calgary Zone clients.
