To support a cooperative healthcare team, the full-time Pennsylvania Licensed RN Case Manager will manage patient-centered care coordination, assess and implement individualized care plans, and monitor health outcomes while working remotely.
Key responsibilities: Assess, plan, implement, coordinate, and evaluate options and services to optimize patient health status Develop and monitor systems of care that promote early intervention in acute care situations Maintain documentation for all case management activities and adjust treatment plans based on collected data Required qualifications: Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania Minimum of 2 years of nursing experience Graduate from a Specialty Training Program in Nursing (Required), Bachelor's Degree in Nursing (Preferred) Certified Case Manager / American Case Management Association (ACMA) certification preferred Knowledge of healthcare laws, regulations, and insurance coverage policies is helpful
