To support a growing telehealth initiative, the full-time Minnesota Licensed Telehealth Physician will build strong relationships with patients and healthcare advocates, oversee patient care plans, and contribute to a recent patient care program in a remote setting.
Key responsibilities Learn and utilize Solace systems and tools while providing clinical expertise in patient interactions Identify and prioritize patients' needs to maintain a streamlined care continuum Oversee patient care plans in collaboration with healthcare advocates addressing social determinants of health Required qualifications Board certification in family medicine, internal medicine, or other relevant specialty Active DEA license and HAP Strong clinical and organizational skills with the ability to manage multiple tasks under pressure Experience working with diverse patient populations, including the elderly and those facing social determinants of health Technical proficiency and adaptability in learning new systems and software
