Coordinating interdisciplinary care plans, the full time Florida Licensed Case Manager RN will work remotely to monitor patient care from admission to discharge, acting as a liaison between patients, families, and healthcare personnel while ensuring compliance with medical advice and insurance requirements.
Key responsibilities Identifies patient risk factors and evaluates care plans through chart reviews and patient interviews Educates patients and families on care progression and facilitates ongoing insurance authorizations Develops discharge plans that address the physical, social, and emotional needs of patients Required qualifications Bachelor's Degree in Nursing Current Registered Nurse (RN) license Minimum of 2 years of relevant experience in case management or nursing
