Seeking a Geriatrician licensed in WA, OR, ID, or OH, the full-time or part time remote position will manage the assessment and longitudinal care of patients with dementia, while leading an interdisciplinary team to enhance patient and caregiver experiences.
Key responsibilities Lead and mentor a multidisciplinary care team to deliver high-value, empathetic patient care Identify improvements in care models and contribute to program evaluation frameworks to enhance dyad outcomes Collaborate with cross-functional teams to translate high-touch care philosophy into clinical requirements and validate new features Required qualifications Board Certified in Internal Medicine or Family Medicine, with a Subspecialty Certification or CAQ in Geriatric Medicine 5+ years of clinical practice experience and an active medical license in at least one state Experience working in interdisciplinary care teams Strong problem-solving skills and ability to navigate challenges creatively Prior experience in a high-growth startup or digital health environment
