To support a clinical engagement team, the part-time contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely with flexible hours between 8:00 am and 8:00 pm CST.
Key responsibilities Engage members telephonically to schedule pharmacist consultation appointments and provide clinical recommendations Document patient care notes and ensure adherence to HIPAA regulations and organizational policies Assess medication regimens for effectiveness and educate patients on their medications and disease states Required qualifications Current and unrestricted Pharmacist license in the state of residence Strong knowledge of disease state management guidelines and the core elements of MTM services 1+ years of experience working remotely/independently in a cross-functional team environment Intermediate proficiency with Microsoft Word, Outlook, and Excel Ability to work a versatile schedule with rotating shifts as needed
