Key responsibilities Assess member needs and develop comprehensive case management plans to improve health outcomes Implement and evaluate interventions to ensure effectiveness in achieving established health goals Collaborate with members, families, and healthcare providers to coordinate necessary resources and services Required qualifications Active and unrestricted RN license to practice in Louisiana Diploma, Associate's, or Bachelor's degree in nursing Three years of recent direct patient care or clinical experience Two years of experience in managed care preferred Certified Case Management Certification preferred, with a requirement to obtain within three years of hire
